    Dear Mark,

I am writing this letter to request you to grant me one month of leave because of some medical emergency has come in my family and I could not able to join office for a month.

Yesterday, my mother has fallen off in the bathroom and her spine got affected and now she is suffering from a slip disc problem. Now doctor prescribed an urgent surgery of the spine otherwise my mother would not able to stand in the future. Her surgery has been scheduled for tomorrow and I need to present there.

In my absence, I will take follow up through my e-mails and be in regular touch with my colleagues for any clarification. I will check my folder regularly so that I would keep updated on office work.

I have informed my colleagues Mr. John and I transferred my responsibilities to him and I will available over the phone if needed and attend any meeting if required.

I looking forward to hearing from you soon and hope for a positive reply from your side.

    Yours Sincerely,

    Jacob
